A Guide To Fill Schengen Visa Application Form

A visa applicant who wants to obtain a visa is required to fill in and sign application form. Filling the forms correctly is one of the most important part of the application process. This guide will help you to understand and fill the forms correctly. Ensure you have your passport, documents and other information with you to fill the forms.

1. Surname (Family name)

Write your surname as indicated in your passport. Your surname is the family name given to you. For example: my name is Samuel Gyapong. Gyapong is my surname and Samuel is my first name.

2. Surname at birth (Former family name)

Write your surname given to you at birth if is different from your surname in question 1. If is the same surname then you can leave question 2 blank or you can write the same the surname there.

3. First name(s) (Given name)(s)

Write the first name and given name(s) as indicated in your passport. For example: my full name is Samuel Asare Gyapong. Samuel is my first name, Asare is my middle name and Gyapong is my surname. So my given names are “Samuel Asare”.

4. Date of birth (day-month-year)

Write the date you were born as indicated in your passport. The day comes first, month in the middle and year at the end. For example: 12-02-1988

5. Place of birth

Write the place you were born as indicated in your passport. For example: I was born in Koforidua, Ghana. So I will write down Koforidua which is the city I was born in.

6. Country of birth

Write down the country where you were born as indicated in your passport. For example, I was born in Ghana so I will write Ghana.

7. Current nationality, Nationality at birth, if different:

Write your current nationality as indicated in your passport.

8. Sex/Gender

Tick the box that apply to you. (Male or Female)

9. Marital Status

Tick the box that apply to you. Single or married or separated or divorced or widow. If other you specify.

10. Minor

Write down the full name, address and nationality of the person holding legal guardianship.

11. National Identity number

Write the number of your national Identity card if applicable.

12. Type of travel document

Tick the type of travel document you traveling with. Any ordinary person might choose ordinary passport. If you have any other travel document than what is on the form then you must specify.

13. Number of travel document

Write down the passport number in your passport. You can find this on the top right side of your passport data page.

14. Date of issue

Write the date the passport was issued. You can find this information on the passport data page.

15. Valid until

Write the passport expiring date. You can find this on the passport data page.

16. Issued by

Write the authority that issued the passport and the place of issue. You can find this on your passport data page.

17. Applicant’s home address, email address, and telephone

Write your address, email address and telephone number.

18. Residence in a country other than the country of current nationality

If you are living in another country other than your current nationality, then you must tick yes, and provide your resident permit number and expiration date.

19. Current Occupation

Write your current occupation.

20. Employers name, address and telephone number/ Name and address of educational establishment

Give your employers name, address and telephone number. Or give your school/university name, address and telephone number.

21. Main purpose(s) of the journey

Tick the purpose of your journey. You can tick more than one. If other, specify.

22. Member State(s) of destination

Write the country you will be residing in the most. They ask you this question because Schengen States issue Schengen visa which allows you to travel to all the Schengen States. Is advisable to write the country you are applying your visa from.

23. Member State of first entry

Specify the country you will be entering the Schengen area. Is advisable to write the same country you are applying your visa from.

24. Number of entries requested

Tick single entry if you want to enter only once. Tick double entries if you want to enter twice. Tick multiple entries if you want to enter multiple times.

25. Duration of the intended stay or transit

Specify how many days you intend to stay. E.g., 90 days, 30days etc.

26. Schengen visas issued during the past three years

Tick no if you haven’t received Schengen visa during the past three years. If yes, specify the issuing date and expiring date.

27. Fingerprints

Tick yes if your fingerprint is collected for the purpose of Schengen visa. And state the date if known.

28. Entry permit for the final country of destination, if applicable

Write the number and validity dates of your final destination, if this country is outside the Schengen area. You can leave it blank if you not traveling outside the Schengen area.

29. Intended date of arrival in the Schengen area

Write the date you intend to arrive in the Schengen area

30. Intended date of departure from the Schengen area

Write the date you intend to leave the Schengen area.

31. Name(s) of the inviting person/Hotel/Sponsor company/Educational institution

Write the name(s), telephone number, address and email of the person inviting you or the hotel/hostel you will be sleeping if you are tourist

32. Name and address of inviting company/organization

Write the name, address, email and telephone number of the company/ organization inviting you.

33. Financial means

You have to specify how you will sponsor your trip. Whether is a self sponsored or someone else will sponsor you. You need to choose either by applicant him/herself or by a sponsor and tick means of support. You can tick as many as possible. And you can also choose both by applicant and a sponsor and you must prove with documents.

34. Personal information of the family member who is EU, EEA or Swiss National

Write the full name, date of birth, Nationality, Number of travel document of your EU, EEA family member.

35. Family relationship

Specify how you are related to the EU, EEA citizen.

36. Place and date

Write the place you applying from and date

37. Signature

The application forms must be signed by the visa applicant and not any other person. If the applicant is a minor, it must be signed by the person who has “legal guardianship and their relationship should be included (father, mother, guardian). A separate application form is required for each applicant”.
